Helpful Booking Tips:
- If this is a medical emergency or you are experiencing chest pain, confusion, or difficulty breathing, CALL 9-1-1 OR HEAD TO THE CLOSEST HOSPITAL

-This form is for patients already registered under our family practice and in our database.   If you are interested becoming a family patient, please email the address below to be put on one of the doctors' waitlists.  We cannot provide medical attention or take responsibility for the healthcare of those not rostered as family patients, even if they have been to the clinic as walk-in patients before.

-Please provide your name and date of birth as they appear on your OHIP card as this is how our database searches for patients

- Please provide as much information about your medical concern as you are comfortable doing so.  This will allow us to triage you accordingly, arrange for an appointment, and get back to you as quickly as possible.
This may include (but is not limited to) date and type of injury, symptoms, severity, how long this has been affecting you, when your prescription finishes, etc

- Prescription Refills: In order for your prescription refills to be covered by OHIP, your doctor needs to speak with you over the phone.  Please contact us BEFORE your prescription runs out so we can ensure your medications are ready in time for your next dose.
